Vidant Health & PMD Healthcare Collaborate to Reduce COPD Readmissions

ALLENTOWN, Pa., July 27, 2017 /PRNewswire/ -- Chronic Obstruction Pulmonary Disease (COPD) is the 3(rd) leading cause of mortality in the United States. Additionally, readmission rates for this patient population are among the highest of any chronic condition. In fact, Vidant Medical Center in North Carolina reports COPD as the 3(rd) leading cause for readmission within it's system.

It is well documented that proper care, planning and education of both the patient and their family can impact readmission and improve quality of life for patients.

With a long reputation to tackle healthcare challenges Vidant Health and PMD Healthcare are collaborating on a COPD readmission reduction program that will be conducted at Vidant's 900+ bed Medical Center in Greenville, NC.

Patients admitted to Vidant Medical Center with a primary diagnosis of COPD will be evaluated upon discharge for inclusion in a remote patient monitoring program utilizing PMD Healthcare's Spiro PD 2.0 and Wellness Management System (WMS) remote monitoring program. Vidant's care team will utilize automated care plans and PMD's WMS Platform to monitor patients' biometrics, medication adherence and symptoms via WIFI enabled technology supporting real time data review by the Vidant team.

Study objectives include:

    1. Evaluation of 30- 60 and 90 day readmissions of COPD patients in the
       study compared to non- study patients.
    2. Evaluation of 30- 60 and 90 day visits to the emergency room of COPD
       patients in the study compared to non- study patients.
    3. Measure medication adherence as assessed by the Spiro 2.0 WMS program.
    4. Demonstrate patient engagement as assessed by per protocol use of the
       Spiro 2.0.
