Texas VSI, LLC ("TVSI") Provides Media Notification Of Data Security Incident

EL PASO, Texas, Nov. 16, 2018 /PRNewswire/ -- On September 17, 2018, Texas VSI, LLC ("TVSI") learned of a security breach at the Ambrosio Guillen Texas State Veterans Home, resulting in unauthorized access to the Veterans Home computer network.

Upon learning of the security breach, TVSI launched an immediate investigation into the full nature and scope of the breach. TVSI discovered the unauthorized access first occurred on May 12, 2018 and was terminated on September 17, 2018. While TVSI continues to review the specific information that may have been accessed, we have determined that some protected health information was stored on the affected server. This information may have included resident name, address, Social Security number, and other health related information such as health insurance numbers and limited treatment records. TVSI has no reason to believe that any of this information was misused. While TVSI continues to identify those residents affected by this incident, we are issuing this media notification in accordance with the Health Insurance Portability and Accountability Act ("HIPAA") federal regulations at 45 C.F.R. 164.406 because TVSI believes that more than 500 residents have been affected.

TVSI has been entrusted to care for Texas veterans and their families and we take the security of our residents' personal information in our care very seriously. Upon discovery of the unauthorized access, TVSI immediately took necessary measures to contain the incident and secure the computer network. TVSI then engaged a third-party forensics firm to determine the full scope of the incident. TVSI has taken many significant steps to ensure the security of the information under our trust. TVSI is notifying all potentially affected individuals and offering them access to thirty-six (36) months of complimentary credit monitoring and identity restoration services with AllClear ID.
