Notice of Data Incident

Houston, TX -- The Harris Center for Mental Health and IDD ("the Harris Center") announces a data incident that may have affected certain individuals' information. On November 7, 2023, the Harris Center experienced a network disruption that affected its ability to access certain files on its network. The Harris Center immediately began an investigation, which included working with third-party specialists. The investigation determined that certain information stored on the Harris Center's network was accessed by an unauthorized individual between November 6, 2023 and November 7, 2023. Therefore, the Harris Center conducted a review to determine the type of information contained on its network. The type of information potentially impacted varied by individual but may include name and one or more of the following: address, phone number, email address, date of birth, Social Security Number, U.S. Alien Registration Number, driver's license/state ID number, financial account information, treatment/diagnosis information, prescription information, provider name, medical record/case number, Medicare/Medicaid ID number, health insurance information, and treatment cost. In response to this incident, the Harris Center notified law enforcement and is reviewing its policies and procedures.
In an abundance of caution, the Harris Center is offering potentially affected individuals access to credit monitoring and identity protection services. To obtain more information about this incident or enroll in these services, individuals should contact the Harris Center's dedicated assistance line at 1-833-972-4120 from 7:00 a.m. to 7:00 p.m. Central time, Monday through Friday, excluding holidays. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion 1-800-680-7289; Experian 1-888-397-3742; Equifax 1-888-298-0045.
Individuals can further educate themselves regarding identity theft, fraud alerts, credit freezes, and steps to protect their personal information by contacting the credit reporting bureaus, the Federal Trade Commission ("FTC"), or their state Attorney General. Instances of known or suspected identity theft should also be reported to law enforcement, your state Attorney General, and the FTC.
