Notice of Data Incident
[Lawrence, KS] - Heartland Community Health Center ("Heartland") announces a data incident that may have affected a limited number of individuals' information. On October 1, 2024, Heartland discovered suspicious activity related to a single employee email account. Heartland immediately began working with third-party specialists to conduct an investigation. The investigation determined that certain information stored in the employee's email account may have been subject to unauthorized access between October 1, 2024, and October 2, 2024. Therefore, Heartland is conducting a comprehensive review of the contents of the information that is potentially affected. On December 17, 2024, they completed their review and then began locating address information to notify individuals. The type of information varies by individual but may include name and one or more of the following data elements: address, phone number, email address, Social Security number, driver's license/state ID number, credit card/debit card information, taxpayer ID number, digital signature, date of birth, medical diagnosis/treatment information, prescription information, date of service, patient ID number, provider name, medical record number, Medicare/Medicaid number, health insurance information, health insurance claim number, health insurance policy number, and/or treatment cost information.
In an abundance of caution, Heartland is offering potentially affected individuals access to credit monitoring and identity protection services. To obtain more information about this incident or enroll in these services, individuals should contact Heartland's dedicated assistance line at 1-833-799-4430, Monday through Friday, from between 7:00 AM and 7:00 PM (CST), excluding holidays. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
