Notice of Data Incident

Montebello, NY - Apex Global Solutions, LLC ("AGS") is providing notification of a data incident. On or around July 14, 2024, AGS discovered potentially suspicious system activity. Upon discovery, AGS took steps to secure its environment in an abundance of caution and immediately conducted an investigation, which included working with third-party specialists. The investigation determined that certain information stored on AGS computers may have been subject to unauthorized access or acquisition between June 18, 2024 and July 2, 2024. Upon discovery, AGS worked to identify the data at risk and conduct a thorough review to determine whether individuals' information may have been at risk, and if so, identify any such individuals to provide notification. On March 17, 2025, this investigation was completed and confirmed the information potentially at risk included individuals' personal information. The information potentially at risk may contain individuals' first and last name together with one or more of the following: address, date of birth, driver's license number, Social Security number, diagnosis/condition, health insurance information, claims information, patient ID, treatment information, provider name and/or financial account information. Upon completion of the investigation, AGS is working to directly notify individuals whose information was identified to have been potentially at risk.
In response to this incident, AGS took immediate steps to secure AGS systems and engage third-party specialists to assist in a thorough investigation and response, as well as implemented additional security measures to further minimize the risk of a similar incident occurring in the future. AGS also notified law enforcement and is reviewing policies and procedures related to data protection. AGS is also providing individuals access to credit monitoring and identity protection services as an added precaution. In general, individuals are encouraged to remain vigilant in regularly reviewing and monitoring all account statements, explanation of benefits statements, and credit history to guard against any unauthorized transactions or activity. Individuals can get one free credit report every year from each of the three major credit reporting bureaus: TransUnion; Experian; and Equifax. To order a free credit report, individuals may visit www.annualcreditreport.com or call 1-877-322-8228.
