Notice of Data Incident

CardioVascular Health Clinic ("CHC") announces a data incident that may have affected certain individuals' information. On March 4, 2025, CHC experienced unusual activity on its network. CHC immediately began an investigation, which included working with third-party specialists. The investigation determined that certain information stored on CHC's network was accessed by an unauthorized individual between February 18, 2025 and March 4, 2025, as a result of a security incident affecting its cloud services provider. Although CHC is unable to confirm the specific information that may be affected at this time, they are providing notification in an abundance of caution as they value the security of information. The type of information potentially impacted will likely vary by individual but may include name and one or more of the following: address, phone number, email address, date of birth, Social Security Number, driver's license/state ID number, financial account information, treatment/diagnosis information, prescription information, provider name, medical record/case number, Medicare/Medicaid ID number, health insurance information, and treatment cost.
In an abundance of caution, CHC is offering potentially affected individuals access to credit monitoring and identity protection services. To obtain more information about this incident or enroll in these services, individuals should contact CHC's dedicated assistance line at 877-648-0819 from 8:00 a.m. to 8:00 p.m. Central time, Monday through Friday, excluding major U.S. holidays. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion 1-800-680-7289; Experian 1-888-397-3742; Equifax 1-888-298-0045.
Individuals can further educate themselves regarding identity theft, fraud alerts, credit freezes, and steps to protect their personal information by contacting the credit reporting bureaus, the Federal Trade Commission ("FTC"), or their state Attorney General.
