Notice of Data Incident 06/26/24

Omaha, Nebraska - Signature Performance, Inc. ("SPI") announces a data incident that impacted some protected health information stored on its network. SPI previously provided healthcare administrative services, including medical coding, insurance resolution, and insurance billing services to Dimmit Regional Hospital, among other organizations.

On January 18, 2024, SPI detected suspicious activity on its network and immediately began an investigation. The investigation included the assistance of third-party specialists and determined an unknown party accessed certain files on SPI's network between January 17, 2024, and January 18, 2024. Therefore, SPI began a comprehensive review of the files at issue to determine the information the files contained and to whom the information related. SPI's review included the assistance of third-party data review specialists and determined the potentially impacted information included the following types of information related to some patients of Dimmit Regional Hospital: name, date of birth, telephone number, medical record number, medical treatment/diagnosis information, dates of service, health insurance provider name, health insurance claim information, and/or treatment cost.

In response to this incident, SPI notified law enforcement and implemented additional security measures to further minimize the risk of a similar incident occurring in the future. While SPI has no reason to believe any of the information described above has been misused, in an abundance of caution, SPI is providing individuals access to credit monitoring and identity protection services. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
