Media Advisory (Ohio) Notice of Data Incident June 20, 2025
Beachwood, Ohio - Retina Associates of Cleveland, Inc. ("RAC") announces a data incident that potentially impacted some protected health information.
On February 5, 2025, RAC identified unusual activity associated with an employee email account and immediately began an investigation with the assistance of third-party specialists. The investigation determined that an unknown party accessed the email account without authorization between January 9 and February 5, 2025, and potentially accessed information contained within the account. Therefore, RAC began a comprehensive review of the account contents to determine the information contained therein and to whom the information related.
The review determined that the type of information potentially impacted, which varies by individual, may include one or more of the following: name, Social Security number, driver's license or state identification number, Medicare or Medicaid number, medical record number, health insurance member number, information relating to diagnosis, treatment, and procedure, medical history or allergies, prescription drugs information, lab test results or images, date of admission or treatment, treatment location, healthcare provider name, cost of treatment, financial account number, street address, phone number, Email address, and date of birth.
In response to this incident, RAC engaged outside cybersecurity specialists to conduct an investigation. RAC also continues to review its policies and procedures related to data protection. RAC has no reason to believe any information has been or will be misused because of this incident. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
